新式外來人口換發新式統一證號變更事宜

Matters related to foreign nationals changing to the new “UI No.”.

內政部移民署自110月1月2日起實施新式外來人口統一證號專案(以下簡稱新式統號)，將原有「2碼英文+8碼數字」外來人口統一證號格式，調整為「1碼英文+9碼數字」之新式證號格式。

On January 2, 2021, the Ministry of the Interior National Immigration Agency (NIA) started implementation of the new UI No. for foreign nationals. The format of the UI No. will change from [2 letters + 8 numbers] to [1 letter + 9 numbers] to become consistent with the National ID format of all R.O.C. citizens.

一、若您為保單關係人（要保人、被保險人、受益人）且已向內政部申請新式證號，提醒您下列事項：

請填寫「變更申請書」，並檢附「最新居留證」正反面影本，送台新人壽辦理。
If you are a party to an insurance policy (policyholder, insured, or beneficiary) and have already applied for the new UI No. with the NIA, please fill in the [Change Application Form] and send it to Taishin Life together with a copy of the front and back of your latest alien resident certificate.

二、若您的保單已辦理授權保險費自動扣繳（銀行自動扣款或信用卡扣款），且授權人已向金融機構申辦更換新式證號，提醒您須同時辦理下列事項：

請填寫「金融機構轉帳代繳保險費授權書」，送台新人壽辦理。
If you have authorized automatic deductions of insurance policy premium payments from your bank account or credit card before, please fill in the [Authorization Form for Insurance Premium Payment by Financial Institution] and send it to Taishin Life at the same time as when applying to replace your new UI No. with the financial institution that you authorized.
